Placing a Loved One
- If you are interested in placing a loved one— your first and most important step is to contact our Admissions Department at Baldwinville Nursing Home at 978-939-2196. Our goal is to ease the transition for you and your loved one.
- Information regarding your loved one will be taken over the phone. We strongly recommend that you make an appointment with our Admissions Staff for a facility tour and additional information.
- Our highly qualified staff will assist you with the admissions process and walk you through the Medicaid application process when your loved one qualifies.
- We work with area hospitals and receive many referrals from them. If your placement preference for your loved one is the Baldwinville Nursing home, please work with the hospital social worker to clarify your preference a priority.
